Standing, Elevating, and Power-Assist Add-Ons: What's Worth It and What Sits in the Closet
I've seen a lot of expensive equipment gathering dust in garages. Not because it was bad equipment β because it was the wrong equipment for that person's actual day.
So let's talk honestly about the three big-ticket add-ons people ask me about most: standing, seat elevation, and power assist. Here's what I've seen work, what I've seen fail, and what I'd tell you if you were sitting across from me.
Power Assist: The Highest Hit Rate of Anything I Spec
If you're a manual chair user with shoulder pain, live somewhere hilly, or push more than a mile or two a day, power assist is usually the best money you'll spend. Shoulder preservation isn't a luxury β a lot of long-term manual users end up with rotator cuff tears and impingement, and once that happens, your independence changes.
But the type matters enormously, and this is where people get it wrong.
Rear-mounted single wheel (SmartDrive): Light (about 12β13 lbs total), doesn't change how your chair handles when it's off, and you keep your normal handrims. Great if you push fine on flat ground but hills and long distances wreck you. Downside: it's push-based control, so precise maneuvering in a tight kitchen is clumsy. Most people learn to just turn it off indoors. Also, it does nothing for you on carpet transitions or when you need help starting from a dead stop on a slope.
Power-assist wheels (Alber e-motion, Twion, Yamaha JWX-2): These read your push force and amplify it. Fantastic for people who need help with every single push β progressive conditions, significant weakness, bilateral shoulder issues. The tradeoff is brutal weight: each wheel is 18β25 lbs. If you load your own chair into a car, these are often a dealbreaker unless you're staying in the chair or have a lift.
Front-mount power add-ons (Firefly, Batec): Genuinely transformative for outdoor life β gravel, grass, hills, long distances at real speed. But they're not indoor equipment, and mounting/dismounting takes time and hand function. These are the ones I most often see used constantly by the right person and abandoned by the wrong one. The right person: someone who wants to go places a wheelchair doesn't go. The wrong person: someone who mostly wants easier grocery shopping.
My rule: if the main problem is distance and hills, get a rear or front add-on. If the main problem is every push hurts, get power-assist wheels and solve the transport problem separately.
Seat Elevation: Underrated, and Now Fundable
Seat elevation used to be nearly impossible to get funded. As of 2023, Medicare covers power seat elevation on power wheelchairs when there's a documented need for transfers or reaching. That changed the math for a lot of people.
What I've seen: elevation gets used daily, quietly, without anyone making a fuss about it. Level transfers to a bed or car seat instead of uphill ones. Reaching a microwave. Talking to a coworker at eye level instead of staring at a belt buckle. It's not dramatic, which is exactly why it works β it fits into the day you already have.
On manual chairs, elevation is a much harder sell. The mechanisms add real weight and often compromise the rigidity and adjustability that make a good manual chair good. If you're in a rigid ultralight and pushing well, I'd usually rather spend that money and weight budget on a better frame, better wheels, and power assist.
Standing: The Highest Rate of Abandonment I See
I want to be careful here, because standing systems change lives for some people. Bone density, bowel and bladder function, spasticity management, contracture prevention, and honestly just the psychological difference of standing at a counter or hugging someone at full height.
But standing has the worst gap between how it feels in a demo and how it fits into real life. Common reasons it goes unused:
- Time. Getting into position, strapping knee blocks and a chest support, standing, and coming back down is a 10β15 minute ritual. People do it daily for two months, then twice a week, then never.
- Space. Standing needs vertical clearance and floor space you may not have in a small apartment or a work cubicle.
- Weight and bulk. A standing power chair is heavier, wider, and harder to transport. A standing manual chair (LEVO, Vertic) is dramatically heavier than a comparable rigid ultralight β you'll feel it every push.
- Orthostatic tolerance. If you get lightheaded standing, you'll stand less than you planned.
My advice: before you commit, borrow or rent a standing frame β a separate, cheap piece of equipment β and use it at home for six weeks. Log it. If you actually stood 30+ minutes a day, five days a week, without anyone reminding you, standing in your chair will get used. If you didn't, put that money into a better cushion, better seating, and power assist.
The Honest Summary
Spend on the thing that solves a problem you have every single day. Power assist and seat elevation usually do. Standing sometimes does, but the burden of proof is on it β and the way to prove it is to try standing separately first.
And whatever you add, remember: no add-on fixes a chair that doesn't fit. A poorly set-up chair with power assist is still a poorly set-up chair, now heavier.
